29 CFR 1926.300(b)(4)(ii): 29 CFR 1926.300(b)(4)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefore, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle.  a) Worksite was located at 798 Broadway, Bayonne, NJ 07002.  Employees were exposed to laceration hazards as they were using an unguarded DeWalt DW818 4 1/2 Inch Angle Grinder to cut ceramic tile roofing for installation on the eave of the building.  Violation was observed on or about March 10, 2020.

29 CFR 1926.451(b)(1): Each platform on all working levels of the scaffold was not fully planked or decked between the front uprights and the guardrail supports:  a) Worksite was located at 798 Broadway, Bayonne, NJ 07002.  Employees were exposed to fall hazards as they were working on a tube and coupler scaffold that was not fully planked while installing ceramic tile roofing on the eave of the building.  Violation was observed on or about March 10, 2020.

29 CFR 1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity:  a) Worksite was located at 798 Broadway, Bayonne, NJ 07002.  Employees were working on a tube and coupler scaffold which contained planks and boards that were dry rotted and cracked through.   Employees were exposed to fall hazards as they were working on a tube and coupler scaffold that was planked with boards that were dry rotted and cracked through. An inspection by a competent person did not identify these visible defects.     Violation was observed on or about March 10, 2020.

29 CFR 1926.451(g)(1): 29 CFR 1926.451(g)(1):  Each employee on a scaffold more than 10 feet above a lower level was not protected from falling to that lower level by fall protection established in accordance with paragraphs (g)(1)(i) through (vii) of this section:    a) Worksite was located at 798 Broadway, Bayonne, NJ 07002.  Employees were exposed to fall hazards as they were working on a tube and coupler scaffold from a height of approximately twelve feet with no guardrails or fall protection while installing ceramic tile roofing on the eave of the building.  Violation was observed on or about March 10, 2020.
