29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level shall be protected from falling to that lower level. Paragraphs (g)(1)(i) through (vii) of this section establish the types of fall protection to be provided to the employees on each type of scaffold. Paragraph (g)(2) of this section addresses fall protection for scaffold erectors and dismantlers.  a. Located at 5254 Lilac Avenue, Grove City, Ohio: On or about March 09, 2020, the employer failed to ensure that an employee was protected from falls while working off a prefabricated (DIRECT SCAFFOLD SUPPLY, Model 5X5 WACO STYLE, B-FS5W51H-D48) scaffold greater than 10 feet.  An employee had performed drywall sanding work at a new build residential home, when he fell off the erected two buck high prefabricated scaffold to the ground level.  The fall distance was greater than 10 feet from the second buck level to the ground level.

29 CFR 1926.21(b)(2): The employer shall instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury.  a. Located at 5254 Lilac Avenue, Grove City, Ohio: On or about March 09, 2020, the employer failed to instruct employees on the avoidance of unsafe conditions while they performed drywall sanding work at a new build residential home.  As a result an employee fell off the erected two buck high scaffold to the ground level.  The fall distance was greater than 10 feet from the second buck level to the ground level.

Located at 5254 Lilac Avenue, Grove City, Ohio: On or about March 09, 2020, the employer did not initiate and maintain a safety and health program to ensure hazard detection and correction, while employees performed dry wall sanding at the worksite.  An employee was unprotected from falling from off the erected two buck high scaffold level to the ground level.  The fall distance was greater than 10 feet from the second buck to the ground level.
