29 CFR 1926.452(j)(2): Poles were not secured to the structure by rigid triangular bracing or equivalent at the bottom, top, and other points as necessary: a) South side, west end area, 101 Centerton Road, Mount Laurel, NJ: Employees were observed working from an improperly setup, approximately 42' long, mixed component aluminum pump jack scaffold; each of the 3 poles were secured with only one rigid triangular brace at the roof approximately 41' above the ground, on or about 03/02/2020; b) South side, west end area, 101 Centerton Road, Mount Laurel, NJ: Employees were observed working from an improperly setup, approximately 15' long, mixed component aluminum pump jack scaffold; each of the 2 poles were secured with only one rigid triangular brace at the roof approximately 41' above the ground, on or about 03/04/2020. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level: a) Southwest corner roof area, 101 Centerton Road, Mount Laurel, NJ: Employees improperly using their personal fall arrest system, approximately 40' above ground level were securing the rigid triangular bracing for two aluminum poles; the rope grabs were not placed to prevent them from falling less than 6 feet before engaging; employees could possibly fall 2-3 times this distance before their fall arrest system would start to engage, on or about 03/02/2020. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
