29 CFR 1926.451(a)(6): Scaffolds were not constructed in accordance with the design by a qualified person: Site: 535 Grand Street in Brooklyn, NY. On or about 02/27/2020. a) Employees were dismantling scaffolding that was not erected, installed and maintained per the Engineer's design. The scaffold had been installed since November 2019 and was not initially constructed in accordance to the design plans and was altered during the until the day of the incident.
