29 CFR 1910.134(f)(2): Employee(s) using a tight-fitting facepiece respirator were not annually fit tested: (a) Pharmacy - On or about March 5, 2020 the employer did not ensure all employees using tight-fitting respirators such as but not limited to 3M, negative pressure, half mask, air purifying respirator were provided a fit test to ensure the respirator would properly fit and prevent employee exposure to respirable hazards.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: (a) Facility - On or about March 5, 2020, the employer did not perform adequate training on the chemicals that employees may be exposed to in the workplace and the employer's exposure control methods for chemicals such as but not limited to Peridox, exposing employees to chemical hazards.
