29 CFR 1926.150(c)(1)(viii): Portable fire extinguishers were not inspected periodically and maintained in accordance with maintenance and use of portable fire extinguishers, N.F.P.A. No. 10A-1970:  On or about March 5th, 2020, and at times prior thereto, a fire extinguisher at the jobsite had not received a periodic maintenance inspection in over 1 year, exposing workers to the hazards of fire.

29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired.  On or about March 5th, 2020, and at times prior thereto, workers were utilizing a portable ladder with structural defects in order to perform work tasks at height, exposing workers to the hazard of falling from height.

29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:  On or about March 5th, 2020, and at times prior thereto, the employer ensure that Respiratory Protection Programs were in place where workers were required to wear N-95 filtering facepiece respirators, exposing workers to the hazards of aggravation of a cardiac or pulmonary disorder, dermatitis, ingestion of a hazardous chemical, or transmittal of disease.
