No. 10A-1970:  On or about March 5th, 2020, and at times prior thereto, a fire extinguisher at the jobsite had not received a periodic maintenance inspection in over 1 year, exposing workers to the hazards of fire.

29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired.  On or about March 5th, 2020, and at times prior thereto, workers were utilizing a portable ladder with structural defects in order to perform work tasks at height, exposing workers to the hazard of falling from height.

29 CFR 1910.134(k)(6): The employer did not provide the basic advisory information on respirators, as presented in Appendix D of 29 CFR 1910.134, in written or oral format to employees who wear respirators when such use was not required by the employer:  On or about March 5th, 2020, and at times prior thereto, workers were utilizing filtering facepiece respirators on a voluntary basis without having been provided basic advisory information regarding respirator usage, exposing the workers to the hazard of dermatitis, aggravation of a cardiac or pulmonary disorder, ingestion of a hazardous chemical, or transmittal of disease.
