A) Extrusion Department, Goodman Cutter Model VAC-U-Torq, Serial #5868 - 25 Howard St, Piscataway, NJ 08854 The employer exposed employees to caught in and amputation hazards in that the employer failed to provide hazardous energy control training to authorized employees who perform maintenance and servicing operations on the Goodman Cutter machines. Condition noted on or about 3/11/2020.
