29 CFR 1910.147(c)(6)(ii): The employer shall certify that the periodic inspections have been performed. The certification shall identify the machine or equipment on which the energy control procedure was being utilized, the date of the inspection, the employees included in the inspection, and the person performing the inspection. The employer is failing to protect the employees from the shock/electrocution hazards resulting from incomplete periodic inspections of the workplace machinery as evidenced in the lack of named employee performing the inspection, and employees included in the inspection. At the jobsite of Watkins Concrete Block located at 14306 Giles Road Omaha, NE 68138 it was found that the annual reviews performed do not list the name of the person performing the inspection and those employees included in the inspection process. This was most recently observed on 7-1-2020.

29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage shall be separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one-half hour. The employer is failing to ensure that oxygen cylinders are separated by firewall, fire barrier, and or distancing of 20 feet. Fire hazards are present as they are not distanced, and a explosive/fire exists that could directly impact the employees working in this area. This was most recently documented on July 1, 2020 at Watkins Concrete 14306 Giles Road, Omaha, NE 68138.
