29 CFR 1926.403(b)(2): Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification: 9320 E VIA DE VENTURA AVE: Temporary lighting for the site was powered using a three-way power receptacle splitter plugged into an extension cord exposing employees to fire hazards. 9320 E VIA DE VENTURA AVE: Employees of a subcontractor engaged in tile work were using a three-way power receptacle splitter plugged into an extension cord, all provided by the electrical contractor, an outdoor setting to power tools onsite, exposing them to electrical hazards.
