29 CFR 1910.28(b)(3)(iv): (iv) Each employee is protected from falling into a ladderway floor hole or ladderway platform hole by a guardrail system and toeboards erected on all exposed sides, except at the entrance to the hole, where a self-closing gate or an offset must be used; Employees were exposed to fall hazards when the opening of the ladderway onto Dock 22, from the trailer staging area, was not equipped with a self-closing gate or offset.

29 CFR 1910.178(n)(4): The driver shall be required to slow down and sound the horn at cross aisles and other locations where vision is obstructed. If the load being carried obstructs forward view, the driver shall be required to travel with the load trailing. On or about March 6, 2020, a forklift operator traveling through the Southeast Intersection of Heavy Gauge 214-34 did not slow down even though stacked material baskets obstructed their vision of the intersection.

29 CFR 1910.178(n)(6): The driver shall be required to look in the direction of, and keep a clear view of the path of travel. On or about March 6, 2020, an employee was driving a Yale forklift toward Dock 22 while carrying a hopper of scrap metal when he collided with a pedestrian. The employee had to look away from their direction of travel in order select and activate a garage door opener for Dock 22.
