Smith, the host employer and outside contractors did not inform each other about their lockout tagout procedures when repairing equipment in the engine room such as but not limited to an Auto Purger.  b) On or about February 29, 2020 and times prior thereto, at OK Foods facility, the host employer and outside contractor did not inform each other about their respective procedures for changing out safety relieve valves in an ammonia refrigeration system.
