Allentown, PA, Facility Front, West Corner - On or about March 9, 2020, employees utilized a portable ladder to access the roof of a commercial building that was not less than 30-foot high, but was not extended at least three feet above the upper landing surface on which the ladder is set.    b) 1880 Steelstone Rd. Allentown, PA, Facility Rear, East Corner - On or about March 9, 2020, employees utilized a portable ladder to access the roof of a commercial building that was not less than 30-foot high, but was not extended at least three feet above the upper landing surface on which the ladder is set.
