OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that critical deficiencies of underhung cranes identified were not promptly corrected, exposing employees to struck by hazards.    a. Tronair Inc., On or about March 12, 2020, at the address of 1 Air Cargo Parkway East, Swanton, Ohio 43558, the employer failed to ensure that a R & M underhung crane, S/N HGW05594 hoist cable/chain was operated in a safe condition.  Annual inspection revealed hoist cable/chain had multiple kinks exposing employees to a struck by hazard.    b. Tronair Inc., On or about March 12, 2020, at the address of 1 Air Cargo Parkway East, Swanton, Ohio 43558, the employer failed to ensure that a Kone underhung crane, S/N 1228308325 hoist cable/chain was inspected at frequent intervals.  Annual inspection revealed hoist cable/chain had welds running on lift wheel, requiring immediate replacement there by exposing employees to a struck by hazard.    c. Tronair Inc., On or about March 12, 2020, at the address of 1 Air Cargo Parkway East, Swanton, Ohio 43558, the employer failed to ensure that a Dayton underhung crane, S/N JM2595WW hoist electric brake was operated in a safe condition.  Annual inspection revealed electric brake requiring cleaning and adjusting exposing employees to a struck by hazard.    d. Tronair Inc., On or about March 12, 2020, at the address of 1 Air Cargo Parkway East, Swanton, Ohio 43558, the employer failed to ensure that a Shawbox underhung crane, S/N K4-79655 hoist electric brake was in operated in a safe condition.  Annual inspection revealed electric brake requiring cleaning, resurfacing and adjusting exposing employees to a struck by hazard.    e. Tronair Inc., On or about March 12, 2020, at the address of 1 Air Cargo Parkway East, Swanton, Ohio 43558, the employer failed to ensure that a CM underhung crane, S/N LW806OST hoist electric brake was operated in a safe condition.  Annual inspection revealed electric brake requiring cleaning and resurfacing exposing employees to a struck by hazard.    Among other methods, one feasible and acceptable method to correct this hazard includes, but is not limited to the following:  American Society of Mechanical Engineers (ASME) developed a standard for Cranes and monorails (with underhung trolley or bridge). B30.17-2015, Chapter 17, perform frequent and periodic inspections and correct any condition that is determined to be a hazard to continued operation.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):    a. Tronair Inc., on or about March 12, 2020, at the address of 1 Air Cargo Parkway East, Swanton, Ohio 43558:  The employer did not ensure employees performing blade changes on a Amanda band saw utilized Specific Energy Control application steps for energy isolation. An operator was exposed to amputation hazards while changing blades on the band saw.     As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lockout/tag-out device application [1910.147(d)(4)], dissipation of residual energy [1910.147(d)(5)(i)], and verification of isolation [1910.147(d)(6)] were not implemented to protect employees from machine servicing hazards.

Tronair Inc., on or about March 12, 2020, at the address of 1 Air Cargo Parkway East, Swanton, Ohio 43558: The employer did not ensure that employees were adequately trained as authorized employees prior to performing blade changes on fixed band saws.  Employee was exposed to amputation hazards while changing blades on the saws while in an energized condition.

Tronair Inc., On or about March 12, 2020, at the address of 1 Air Cargo Parkway East, Swanton, Ohio 43558: In the main shop, a temporary extension cord was daisy chained with a relocatable power tap to supply power to a   Delta Mider Saw, Model 50-723-T2,  17C 101146 2017 13 MT and wood dust collector exposing employees to a shock hazard.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure:   a. Tronair Inc., On or about March 12, 2020, at the address of 1 Air Cargo Parkway East, Swanton, Ohio 43558:  In the main shop, the employer used a temporary power cord as a substitute for fixed wiring in that the  flexible cord was used on a continual basis to power a Delta Miter Saw, Model 50-723-T2,  S/N 17C 101146 2017 13 MT and wood dust collector there by exposing employees to an electrical hazard.
