29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:   At the jobsite, employees ascended and descended from the rooftop on a Werner aluminum extension ladder, which did not extend at least three feet above the rooftop landing, exposing the employee to fall hazards greater than 6 ft. to the ground below.  a) Ascending and descending the rooftop on Building 3.  b) Ascending and descending the rooftop on Building 4.  c) Ascending and descending the rooftop on Building 6.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: (Construction Reference: 1926.59)   At the job site, the employer did not ensure that employees exposed to hazards associated with using   SRS Top Shield Hybrid Bonding Sealant, while applying it to roof flashing were provided effective information and training on hazardous chemicals in their work area at the time of their initial training. This condition exposed employee(s) to dermal and inhalation hazards.   a) Applying sealant on Building 3.   b) Applying sealant on Building 4.   c) Applying sealant on Building 6.
