29 CFR 1926.1053(b)(6): Ladders were not used only on stable and level surfaces unless secured to prevent accidental displacement: a) Job site, 5180 Campbells Run Road, Pittsburgh, PA: On or about March 12, 2020, during construction activities, the employer did not ensure that an extension ladder was used safely in that it sat on top of a piece of wood which crossed over an earthen depression near the building's foundation and the wood was not secured to prevent movement.
