29 CFR 1910.22(c): The employer did not provide, and ensure each employee used, a safe means of access and egress to and from walking-working surfaces. (a) Finishing Department: On or about March 13, 2020, employee's working in and around the Northeast end of the Acid Wash area were not provided an unobstructed means of egress and entry to the work area. An extension ladder leaned against the wall and extending into the walkway next to the end of a concrete panel, creating an obstructed egress route from the acid wash area to exits and other work areas/spaces.

(a) Finishing Department: On or about March 13, 2020, adequate inspection of aisles, passageways, and work areas that change regularly based on the size, shape, and configuration of the semi finished products brought in, and the location of tools, equipment and materials needed to process the work pieces were not conducted in a manner that insures a safe work area for employees was maintained.

29 CFR 1910.179(n)(2)(iii): The employer did not ensure that care was taken to prevent the sling from contacting obstacles. (a) Finishing Department: On or about March 13, 2020, the employer did not take effective precautions to insure the eight (8) foot long, 2 chain sling connected to the 40 ton capacity crane would clear obstacles in the path of travel. During rigging of a concrete panel, the chain sling was raised by the crane hoist, which caught on a metal gusset cast into back side of the panel, and caused the panel to lift slightly and flip off the A- frame concrete panel stand. This exposed employees to crushing hazards.

(a) Finishing Department: On or about March 13, 2020, at the 40 ton capacity overhead bridge crane used to move concrete panels in, around, and out of the concrete finishing department, employee(s) placed their hand(s) on the load while guiding the concrete panels into, and out of the three (3) aisles that make up the department, and then onto the flatbed trailers used to transport the panels from the facility. This exposed employees to crushing hazards.
