29 CFR 1910.219(d)(1): Guarding. Pulleys, any parts of which are seven (7) feet or less from the floor or working platform, were not guarded in accordance with the standards specified in paragraphs (m) and (o) of this section. Pulleys serving as sheave wheels on which the point of contact between belt and pulley is more than six feet six inches (6 ft. 6 in.) from the floor or platform were not guarded with a disk covering the spokes:  a)  Cartel Drilling, LLC, TeVelde 28-3HZ, WCR 17/18, Firestone, CO  80504:  On or before March 8, 2020, Cartel Drilling did not guard the pulldown sheave assembly  located on the working platform where drilling operations take place. During tripping out of hole (TOOH) operations, an employee reached over to grab the tong line to keep it from entangling with the pulldown sheave assembly. As the employee was grabbing the tong line, the driller actuated the pull down sheave which resulted in the employee's hand being pulled into the unguarded pull down sheave assembly.  This condition resulted in an amputation  of an employee's two fingers.

29 CFR 1904.39(a)(2): Basic Requirement. Within twenty-four (24) hours after the an employee's amputation as a result of a work-related incident, the employer did not report the amputation to OSHA:   a)  Cartel Drilling, LLC, TeVelde 28-3hz, WCR 17/18, Firestone, CO  80504:  On or  before March 8, 2020, Cartel Drilling did not report a amputation to OSHA within twenty-four (24) hours after an employee received a amputation as a result of a work-related incident.  During tripping out of hole (TOOH) operations, an employee reached over to grab the tong line to keep it from  entangling with the pulldown sheave assembly.
