Employee(s) were exposed to eye and face hazard(s).

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  F Unit, Inmate Preferred Housing Security Office: One refrigerator and one microwave used for employees breaks were plugged into a relocatable power tap. Employees were exposed to fire and/or electrical shock hazards from using electrical equipment improperly.

29 CFR 1910.303(g)(1)(ii): The required working space about electric equipment rated 600 volts, nominal, or less to ground, was used for storage:  Welding Shop Electrical Panel: Employer permitted welding equipment to be directly stored in front of the electrical panel.  This condition prevented access and immediate shutdown of a circuit to the panel, particularly in the event of an emergency.  This condition exposed employees to an electrical and/or fire hazard.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws:  F Unit Inmate Preferred Housing East Hall Way: The employer did not ensure that the wiring connection point of the industrial fan housing and outlet plug were provided with a strain relief.  The industrial fan(s) cord entering the metal casing and the outlet plug point did not have protection to prevent pull being directly to the joints and terminal screws which exposed wiring in this instance. This condition exposed employees to electrical shock hazards.
