29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc.  a) Gregory Log and Lumber Ltd, located at 5545 County Road 68, Millersburg, OH 44654: On and or preceding March 17, 2020, the employer did not ensure that employees were protected from rotating parts. Employees operated the diesel engine, which powered the hydraulics of the de-barker machine, where the rotating parts between the clutch and hydraulic pump were not guarded, thereby exposing the employees to a caught-in hazard.

All elevated walks, runways, or platforms, if 4 feet or more from the floor level, shall be provided with a standard railing except on loading or unloading sides of platforms. If height exceeds 6 feet, a standard toe board also shall be provided to prevent material from rolling or falling off.   a) Gregory Log and Lumber Ltd, located at 5545 County Road 68, Millersburg, OH 44654: On and or preceding March 17, 2020, the employer did not ensure that employees working on elevated walks, runways, or platforms were protected from falls. Employees used an elevated platform to access the control room for the de-barker machine where there was no railing on one side of the platform and no toe boards on any sides, thereby exposing employees to a 7 foot fall hazard.

29 CFR 1910.265(c)(5)(ii): Handrails. Stairways shall be provided with a standard handrail on at least one side or on any open side. Where stairs are more than four feet wide there shall be a standard handrail at each side, and where more than eight feet wide, a third standard handrail shall be erected in the center of the stairway.  a) Gregory Log and Lumber Ltd, located at 5545 County Road 68, Millersburg, OH 44654: On and or preceding March 17, 2020, the employer did not ensure that handrails were installed on the stairway of the de-barker machine. Employees used a stairway, without handrails, to access an elevated platform for the control room of the de-barker machine, thereby exposing employees to a 7 foot fall hazard.
