29 CFR 1910.132(f)(3)(iii): The employer did not retrain employees when inadequacies in the affected employee's use of PPE indicated the employee had not retained the requisite understanding.  Bureau of Prisons, Recycling Plant, Toner Disassembly Area:  Employees perform computer and toner disassembly for recycling.  Employees were trained to wear dust masks and were not authorized to wear half face respirators with organic filters.  In this instance, an employee was observed wearing a half face respirator.  The employer did not ensure adequate retention of training provided ensured PPE selection for tasks was retained by the employee.  The employee was exposed to respiratory hazards.
