29 CFR 1910.217(c)(2)(i): It shall prevent entry of hands or fingers into the point of operation by reaching through, over, under or around the guard. In the brake press machine area, employees engaged in cutting metal parts with an unguarded hydraulic brake press machine were exposed to caught-in and amputation hazards when reaching into the pinch point/cutting area.

29 CFR 1910.217(c)(2)(vii): Point of operation enclosures on mechanical power press(es), which did not meet the requirements of this subparagraph and Table O-10, were not used in conjunction with point of operation devices. In the brake press machine area, the employer did not ensure a hydraulic brake press machine met point of operation guarding based on the requirements of table O-10 when employees were cutting material at or over 1/4 inch width.
