29 CFR 1926.501(b)(1):   Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  At a commercial job site located at 5743 Paseo Blvd, Kansas City, MO. Employees were exposed to serious injuries, in that the unprotected sides and edges of the elevated work area was not provided with guardrails or any other means of fall protection. The employees were exposed to an approx. 13  ft fall hazard to west and 22 ft fall hazard on the eastside  to the concrete surfaces below.

An employee was exposed to a fall hazard by using a ladder which was placed on the back of a tailgate of a pickup truck. Surface was not level or stable for use by employees accessing upper level of roof by ladder.
