29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees: a) Millwork area, heavy duty slat saw; on or about March 18, 2020 and at times prior to, the employer allowed employees to conduct maintenance activities on the heavy duty slat saw such as blade change and jam clearing out without first providing training on the principles of energy control exposing the employees to hazards associated with the control of hazardous energies.

29 CFR 1910.213(n)(3): Feed rolls were not guarded by a hood or suitable guard to prevent the hands of the operator from coming in contact with the in-running rolls at any point. The guard were not fastened to the frame carrying the rolls so as to remain in adjustment for any thickness of stock: a) Millwork area, Heavy duty slat saw; On or about March 18, 2020 and at times prior to, the employer allowed employees to operate the Heavy Duty Slat Saw with the guard propped open exposing employees to amputation hazards.
