29 CFR 1910.213(s)(9): Suitable sizes and types of push sticks or push blocks were not provided at the work place: On or about January 27, 2020 at the worksite located at 418 Hwy 441 Business in Cornelia, Georgia, within the maintenance building, an employee received an amputation to his right index finger while operating a Delta Shopmaster table saw to cut stock. Th employer failed to ensure that adequate push sticks and/or blocks were available to the employee utilizing the table saw. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. On or about 3/9/2020, at the worksite located at 418 Highway 441, in Cornelia, GA: The employer failed to report to OSHA a work-related amputation on or within 24 hours of such knowledge and did not contact OSHA of the following injury: On or about 1/27/2020, an employee operating a table saw received an amputation to their right index finger. OSHA became aware of this employee amputation injuries on 2/27/2020. No abatement certification or documentation is required for this item.
