29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: a) On or about March 24, 2020, and at times prior thereto, keys were left in the ignition, in the "on" position, with the engine running, of an unattended Mack dump truck, model CH613, while servicing and maintenance was performed, exposing employees to crushed-by hazards. NOTE: Abatement certification AND supporting documentation of corrective actions are required.
