29 CFR 1926.451(a)(6): Scaffolds were not designed by a qualified person and were not constructed and loaded in accordance with that design: On or about 27 March 2020, and at times prior thereto, a job-made personnel work platform attached to rough terrain truck, Model 509-42, was used to lift personnel performing remodeling activities without the platform being designed by a qualified person, exposing workers to a fall hazard of approximately 18 feet to ground below.

29 CFR 1926.602(c)(1)(ii): Modifications or additions which affected the capacity or safe operation of the equipment were made without the manufacturer's written approval. On or about 27 March 2020, and at times prior thereto, a job-made personnel work platform attached to rough terrain truck, Model 509-42, was used to lift personnel for remodeling activities where the box was unstable and exceeded the wheel base of the lift, exposing workers to fall and struck-by hazards.
