On or about December 30, 2019, at the above addressed jobsite, employees of a contractor performing demolition operations, accessing the catwalk, were exposed to falls due to the catwalk being damaged. The employer did not inspect to determine the structural integrity of the catwalk.�

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. a. On or about December 30, 2019, at the above addressed jobsite, employees performing demolition operations from a catwalk on the northside of building were exposed to falls, of forty-one (41) feet, due to missing and/or damaged guardrails on the catwalk.
