The employer was required to submit abatement within 15 working days of receiving the violation.

29 CFR 1910.178(l)(1)(ii): The employer did not ensure that each operator had successfully completed the training required by paragraph (l), except as permitted by paragraph (l)(5), prior to permitting an employee to operate a powered industrial truck:  At a jobsite located at 2931 Northeast Independence Avenue, Lees Summit, Missouri 64064, employees were exposed to serious struck-by hazards. The jobsite foreman was observed operating a Skytrak all-terrain forklift without first receiving the required operator training and evaluation for the equipment.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  At a jobsite located at 2931 Northeast Independence Avenue, Lees Summit, Missouri 64064, employees were exposed to serious fall hazards. Employees were working from elevation without the use of adequate fall protection systems in place. Employees were exposed to fall hazard distances of 13 to 18 feet to the ground below.

29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  At a jobsite located at 2931 Northeast Independence Avenue, Lees Summit, Missouri 64064, employees were exposed to serious eye and face injuries. Employees were utilizing assorted hand tools while working at elevation. The employer did not ensure that each affected employee used with appropriate eye or face protection.
