29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following:  guardrail systems, safety net systems, or personal fall arrest systems:     A)    Cooper Farms Processing, located at 1 Cooper Farm Drive, Saint Henry, OH 45883: On or about March 25, 2020, the employer did not ensure that employees were protected from fall hazards while working on the platform located in the Picker Room. The front of the platform (entry point) was not adequately protected by the guardrail/mid-rail system or other acceptable methods of fall protection. Employees were cleaning blood from the platform and were exposed to the fall hazards of approximately 7 feet from the platform to the ground below.

29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):     A)    Cooper Farms Processing, located at 1 Cooper Farm Drive, Saint Henry, OH 45883: On or about March 25, 2020, the employer did not ensure that the point of operation of the S.B. IMPIANTI Tail Remover (Model: MSC-1; Serial # 2011SB02), located at the platform in the Picker Room, was adequately guarded. Employees were exposed to caught-in-between hazards that could result in serious injuries while working in a close proximity to the Tail Remover while it was running.

29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven (7) feet or less above floors or platforms were not enclosed:  A)    Cooper Farms Processing, located at 1 Cooper Farm Drive, Saint Henry, OH 45883: On or about March 25, 2020, the employer did not ensure that one or more methods of adequate machine guarding were provided to protect employees from hazards such as, but not limited to, the nip-point created between the chain and the sprocket of the S.B. IMPIANTI Tail Remover (Model: MSC-1; Serial # 2011SB02) located at the platform in the Picker Room. Employees were exposed to nip-point hazards that could result in serious injuries.
