29 CFR 1910.178(m)(5)(i): When a powered industrial truck was left unattended, load engaging means were not fully lowered, controls were not netralized, power was not shut off, and brakes set. On or about April 6th, 2020, a telescopic forklift that was being used in support of a demolition project was left unattended with the load engaging means raised and extended, exposing workers to the hazard of being struck by the vehicle or supported load.

29 CFR 1926.502(f)(2)(ii): The rope, wire, or chain was not rigged or supported in such a way that its lowest point (including sag) was not less than 34 inches (0.9 m) from the walking/working surface and its highest point no more than 39 inches (1.0 m) from the walking/working surface. On or about April 6th, 2020, and at times prior thereto, the fall protection warning lines for demolition activities occurring on a rooftop had a high point of 50" from the walking/working surface, exposing workers to the hazard of falling from height.
