29 CFR 1910.29(b)(1): (1) The top edge height of top rails, or equivalent guardrail system members, are 42 inches (107 cm), plus or minus 3 inches (8 cm), above the walking-working surface.  The top edge height may exceed 45 inches (114 cm), provided the guardrail system meets all other criteria of paragraph (b) of this section (see Figure D-11).    On or about 10 April 2020, the employer did not ensure that the guardrail around the transfer pit was at the standard requirements of 42 inches plus/minus 3 inches. This exposed employees to a fall hazard.     Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

Employees were not required to isolate and render equipment inoperable before performing maintenance, servicing, and cleaning operations on all company equipment to include conveyor systems.    Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

Employees gaining access to the machines who perform servicing, maintenance, and cleaning operations were exposed to caught in hazards. Kent Rd, Pearl City, IL. 61062.
