29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s):  On or about March 10, 2020, and at times prior thereto and at times thereafter, the employer did not provide a workplace were any employee, worker, laborer, or mechanic employed in the performance of the contract work was free from recognized hazards exposing employees and workers to the following hazards:     scaffolds were not erected safely, a ladder was not installed, and guard rails or fall protection was not used exposing employees/workers to fall hazards.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  On or about March 10, 2020 and at times prior thereto, the employer did not ensure all scaffold systems were erected and safe for use prior to allowing employees/workers on the scaffold from being exposed to unprotected fall hazards.  Guard rails systems were not installed and personal fall protection was not being used.

On or about March 10, 2020, and at times thereafter, the employer did not report to OSHA of an in-patient hospitalization from a work related injury within the required 24 hours or thereafter.
