Employees engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest systems: On 4/13/2020, Brian Dawson Roofing LLC exposed sub-contractor employees to fall hazards while installing shingles on a residential building located at 226 Fox Haven, OFallon, Missouri. Employees were installing shingles approximately 20 feet above a lower level. Employees were not utilizing guardrail systems, a safety net system, or a personal fall arrest system. The employees were observed on the roof of the building without fall protection.
