29 CFR 1926.1204(b): The employer did not identify and evaluate the hazards of the permit required confined space before employee entry: On or about April 15, 2020, inside the riser of the 250,000-gallon multi-column elevated water storage tank, employees were performing tasks such as but not limited to rolling/painting welds and sharp edges, sandblasting and blowing sand. The employer did not perform a pre-entry assessment of the permit spaces before employees entered. Employees were exposed to hazards associated with inhaling respirable dust and chemical vapors: a) Inside the bowl/tank where employees were sandblasting and blowing sand. b) Inside the riser where employees were rolling/painting welds and sharp edges.
