29 CFR 1926.451(e)(1): When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface shall be used. Crossbraces shall not be used as a means of access.  On or about February 20, 2020, employees using ladder jack scaffolding were not provided with a safe means of access and egress to the work platform.  An separate access ladder was not provided, requiring employees to climb around the work platform for access and egress.   Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold shall be protected by a personal fall arrest system.  On or about February 20, 2020, the employer failed to ensure that employees were protected from fall hazards while working on a ladder jack scaffold approximately 30 feet above grade.  Fall protection was not being utilized.   Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

29 CFR 1926.452(k)(1): Platforms shall not exceed a height of 20 feet (6.1 m).  On or about February 20, 2020, an employee was exposed to fall hazards while working on ladder jack scaffold approximately 30 feet above grade.  The work platform of the scaffold exceeded a height of 20 feet.    Abatement certification is required for this item in accordance with 29 CFR 1903.19(c).

On or about February 20, 2020, an employee was exposed to fall hazards while working on ladder jack scaffold approximately 30 feet above grade.  The work platforms were bridged to one another over a single support ladder.     Abatement certification is required for this item in accordance with 29 CFR 1903.19(c).

29 CFR 1926.454(a): The employer shall have each employee who performs work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. The training shall include the following areas, as applicable:  On or about February 20, 2020, employees using ladder jack scaffolds were not provided with training to ensure they were able to recognize and control the hazards associated with scaffold use.  Employees were exposed to fall hazards while working from the scaffold.   Abatement certification is required for this item in accordance with 1903.19(c).
