29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, (c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D) of this standard: Worksite: The employer did not ensure through establishing not only procedures that clearly and specifically outlined the scope, purpose, authorization, rules, and techniques, but also specific procedural steps for shutting down, isolating, blocking and securing machines or equipment so that all hazardous energy sources were under control while two (2) employees were servicing, maintenance and/or setting up a Delta Shear (machine). On or about April 20, 2020, while the two (2) employees were setting up the machine to perform normal production operation, the feeder rollers of the machine came down on both employee's hands, resulting severe hand injuries.
