The employer did not develop and implement a written respiratory protection program. ABATEMENT CERTIFICATION IS REQUIRED FOR THIS VIOLATION

The employer was aware of the diagnosis. As of August 26, 2020, the host employer did not record the employee's illness in their OSHA 300 logs. b) At the facility - 3025 Chapel Avenue West, Cherry Hill NJ: On April 17, 2020, a Licensed Practical Nurse (LPN) tested positive for COVID-19. The employer was aware of the diagnosis. As of August 26, 2020, the employer did not record the employee's illness in their OSHA 300 logs. c) At the facility - 3025 Chapel Avenue West, Cherry Hill NJ: On or about April 17, 2020, a Certified Nurse Assistant (CNA) tested positive for COVID-19. The employer was aware of the diagnosis. As of August 26, 2020, the employer did not record the employee's illness in their OSHA 300 logs. d) At the facility - 3025 Chapel Avenue West, Cherry Hill NJ: On or about April 17, 2020, a Certified Nurse Assistant (CNA) had a COVID-19 positive result.
