29 CFR 1910.28(b)(15): Walking-working surfaces not otherwise addressed. Except as provided elsewhere in this section or by other subparts of 29 CFR part 1910, the employer must ensure each employee on a walking-working surface 4 feet (1.2 m) or more above a lower level is protected from falling by: (i) Guardrail systems; (ii) Safety net systems; or (iii) Personal fall protection systems, such as personal fall arrest, travel restraint, or positioning systems. a) On 4/23/20 the employer did not ensure employees working to clear a jam on the Signode Plastic Strapping Machine (AM 514 Z-20), were protected from fall hazards in that during the process of clearing the jam an employee climbed up on a stack of pavers (approx. 57" above a concrete floor) that were sitting on a conveyor system exposing the employee to fall from approximately 57".

29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: (1) authorized employee shall have knowledge of the type and magnitude of the energy, the hazards of the energy to be controlled, and the method or means to control the energy. (2) turn off and shut down the machine (3) physically locate and operate the energy isolating devices in such a manner as to isolate the machine or equipment from the energy sources. (4) affix lockout or tagout devices to each energy isolating device (5) Prior to starting work on machines that have been locked out or tagged out, verify that isolation and deenergizing of the machine or equipment have been accomplished. a) On 4/23/20 an employee was in the process of clearing a jam on a Signode Plastic Strapping Machine (AM 514 Z-20), the employee climbed up on a stack of pavers (approx. 57" above a concrete floor) that were sitting on a conveyor system, laid down under the head of the bander, the machine activated bringing the head down on the employee.
