This was demonstrated when employees were engaged in shingle replacement work and were not protected from falling by the use of guardrail systems, safety net system, or personal fall arrest system.

This was demonstrated when employees were engaged in shingle replacement work and using a folding step ladder that was not fully opened and locked, which was not for the purposes for which it was designed.
