29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames,  or uprights did not bear on base plates, mud sills or other adequate firm foundation:  At a jobsite located at lot 5, North Tullis Drive and Northeast 102nd street, Kansas City, Missouri 64157, an employee was exposed to a serious fall hazard. An employee was observed working from a supported scaffold. The supported scaffold legs were not placed on adequate firm foundation. The employee was exposed to a fall hazard distance of 10 feet 10 inches to the ground below.

29 CFR 1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity:  At a jobsite located at lot 5, North Tullis Drive and Northeast 102nd street, Kansas City, Missouri 64157, an employee was exposed to a serious fall hazard. A competent person did not inspect the scaffold that the employee was utilizing for visible defects before the start of their work shift. The employee was exposed to a 10 foot 10 inch fall hazard to the ground below.

An employee was observed working from a supported scaffold. The employee was not protected from falling with adequate means of fall protection. The employee was exposed to a fall hazard distance of 10 feet 10 inches to the ground below.

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed:  At a jobsite located at lot 5, North Tullis Drive and Northeast 102nd street, Kansas City, Missouri 64157, an employee was exposed to a serious fall hazard. An A-frame ladder that an employee was observed utilizing was not properly being used as designed by the manufacture. The A-frame ladder was closed and propped up against the scaffold as the employee utilized it to get down to the ground level. Fall hazard was measured at 10 feet 10 inches to the ground below.
