29 CFR 1926.20(b)(2): The employer did not ensure that such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers. On or about April 27, 2020 and at times prior thereto, Centre Living Homes as the Controlling Employer did not ensure that: a) Employees of its subcontractor were not allowed to work from a powered industrial truck with an unapproved work platform. Employees were potentially exposed to falls of more than 20 feet to the concrete below; and b) Its employees were exposed to being struck by falling debris from an unapproved wooden box or being struck by by collapsing components of that unapproved wooden work platform; and c) Employees of its subcontractor were not protected from falling more than 10 and 20 feet to the ground and/or concrete below by ensuring the use of fall protection.
