29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use:  This violation occurred on June 25, 2019, where at least two (2) employees working from a job made construction wooden box that was attached to the forks of a JLG Skytrak front end loader and the construction box was not specifically designed by the manufacturer for such use, exposing the employees to a fall of more than 30 feet.

29 CFR 1926.501(b)(1):  The employer did not ensure that each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level was protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  This violation occurred on June 25, 2019, where at least two (2) employees working unprotected sides or edges were exposed to falls of approximately 10 and 20 feet to the ground below.  a)    One employee worked from an unprotected side or edge while on a balcony approximately 10 feet to the ground below;  b)    One employee worked on a roof with an unprotected side or edge and was exposed to a fall of approximately 20 feet to         the ground below.
