29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes (including skylights) by covers: (a) In the roadway, on the jobsite: On or about 4/29/2020, there were two unprotected 12' circular open holes in the roadway. The first 12 inch hole was estimated to be approximately 7 feet deep. The second 12 inch circular hole was measured to be approximately 10 feet 8 inches deep. Neither hole was covered, or had a guardrail system erected around the hole, to prevent employees from stepping into them.
