29 CFR 1910.134(f)(2): Employee(s) using tight-fitting facepiece respirators were not fit tested prior to initial use of the respirator or annually refitted: a) Department of Veterans Affairs, Veterans Health Administration, located at 4545 East 9th Avenue, Office Building 1, Suite 010, Denver, CO 80220: On or about April 27, 2020, the employer did not provide a fit test to all employees required to wear respirators. The employer required employees to wear N95 filtering facepiece respirators to protect against the SARS-CoV-2 virus while providing care to suspected and confirmed positive COVID-19 patients. The Department of Veterans Affairs was previously cited for a repeat violation of this occupational safety and health standard or its equivalent standard 1910.134(f)(2), which was contained in OSHA inspection number 1116115, notice number 1, item number 1 and was affirmed as a final order on 6/8/2016, with respect to a workplace located at 800 Zorn Ave, Louisville, KY. The Department of Veterans Affairs was previously cited for a repeat violation of this occupational safety and health standard or its equivalent standard 1910.134(f)(2), which was contained in OSHA inspection number 1140139, notice number 1, item number 1 and was affirmed as a final order on 6/6/2016, with respect to a workplace located at 3801 Miranda Ave. Palo Alto, CA.
