29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    Thermodyn Corporation Inc., Sylvania, Ohio: On or about April 27, 2020, the employer did not provide adequate guarding to prevent employee exposure to the ingoing nip point created on the wind up machine on Michigan Press.  Employees working on or near the exposed nip point were exposed to caught in hazards.

29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven (7) feet or less above floors or platforms were not enclosed:  Thermodyn Corporation Inc., Sylvania, Ohio: On or about May 5, 2020, the employer did not provide guarding around a vertical chain and sprocket on the wind up machine located on the Michigan Press.  Employee(s) operating on or near the unguarded chain and sprocket of the wind up machine were exposed to caught in hazards.

29 CFR 1904.29(b)(1): The employer did not provide the required injury or illness descriptions on the Log of Work-Related Injuries and Illnesses, OSHA Form 300:     Thermodyn Corporation Inc., Sylvania, Ohio: On or about May 5, 2020, the employer failed to ensure that the OSHA Form 300 for calendar year 2020 contained a complete description in column F.  The 2020 300 log did not define the specific injury sustained by the employee.
