29 CFR 1926.502(d):  Personal fall arrest systems and their use did not comply with the provisions set forth in paragraphs (1) through (24) of this section:    a.    On or about April 29, 2020, three roofing employees installing roofing shingles on 7:12 pitch roof levels were exposed to 9 to 19 feet fall hazards in that the lanyards were not attached to D-rings, exposing them to a fall hazard.
