ESTABLISHMENT: The OSHA 300 log of Work-Related Injuries and Illnesses for the following years were not completed in details as follows: 1. On or about February 25, 2019: Unknown Case Number; No entry made in Column I, Job Transfer, where an employee had 10 days job transfer from an elbow injury. 2. On or about December 2, 2019: Unknown Case Number; No entry made in Column I, Job Transfer, where an employee had 22 days job transfer from a back sprain injury. 3. On or about January 16, 2020: Unknown Case Number; No entries made in Column H, Days Away from Work, and Column K, Days On Job Transfer, where an employee had 2 days away from work and 2 days job transfer from a back sprain injury.
