29 CFR 1926.451(e)(1): When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface were not used. Crossbraces were used as a means of access: On 5/1/2020, employees from St. Charles Glass & Glazing, Inc. were installing glass and associated hardware onto a commercial building located at 317 West Pearce Boulevard, Wentzville, MO. Employees did not have sufficient scaffold access to the upper working levels of the scaffold system. An Employee was climbing on the scaffold crossbracing as a means of access/egress.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level was not protected from falling to that lower level: On 5/1/2020, employees from St. Charles Glass & Glazing, Inc. were installing glass and associated hardware onto a commercial building located at 317 West Pearce Boulevard, Wentzville, MO. The employees were working from a tube frame scaffold system approximately 11 feet above the ground. Employees were not protected from falling to the lower level.

29 CFR 1926.454(a): The employer did not have each employee who performs work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards: On 5/1/2020, employees from St. Charles Glass & Glazing, Inc. were installing glass and associated hardware onto a commercial building located at 317 West Pearce Boulevard, Wentzville, MO. Employees were not trained by a qualified person in hazard recognition for the tube fame scaffold being used. The employees did not show competence in hazards associated with working at heights on a scaffold.
