29 CFR 1926.416(a)(3): Before work was begun the employer did not ascertain by inquiry or direct observation, or by instruments, whether any part of an energized electric power circuit, exposed or concealed, was so located that the performance of the work could bring any person, tool, or machine into physical or electrical contact with the electric power circuit; the employer did not post and maintain proper warning signs where such a circuit existed; and the employer did not advise employees of the location of such lines, the hazards involved, and the protective measures to be taken: On or about May 1, 2020, employees were conducting electrical demolition operations on the load side of the fifth floor electrical disconnect in the building being remodeled while it was energized, exposing employees to the hazard of being electrocuted, shocked or burned.

29 CFR 1926.416(d): Special tools insulated for the voltage were not used while installing or removing fuses with one or both terminals energized: On or about May 1, 2020, an employee removed the fuses from a 480V energized service disconnect during electrical demolition operations on the fifth floor of the building being remodeled without utilizing insulated tools, exposing the employee to the hazard of being electrocuted, shocked or burned.
