29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:    Location: 17 Richardson Ave., Wakefield, MA    On or about 04/15/2020, workers were not adequately instructed in the recognition and avoidance of unsafe conditions while working from a home-made scaffold and exposed to fall hazards such as;  1. Fall hazards of greater than 10-feet while working from a homemade scaffold without fall protection.  2. Fall hazards while working from an inadequately constructed and erected homemade scaffold. The scaffold collapsed resulting in two workers falling to the ground below, one of which was hospitalized for eight days.  3. Workers did not receive fall protection training.
